Cape Fear Games

I recently moved to the area from OH, and I don't think my DCI card made the journey with me.

I played twice with it, only placed 8 out of 16 in an M10 draft and placed 2nd in the Zendikar Pre-release tourny out of 24. How do I go about getting a replacement card/how do I figure out my DCI number?

Also, would it be easier to just sign up for another DCI card?
